Passport Service

The Birmingham Public Library offers passport services at Central, Avondale Regional Branch, and Springville Road Regional Branch libraries.

New & Renewal Applications

We assist with both new passport applications and renewals.

How to Apply

You have three options for applying for a passport:
  • Fill it out online and print the completed form
  • Print a blank copy and fill it out by hand
  • Complete the form at the library during your visit

Important: Do not sign the DS-11 form until instructed by a passport acceptance agent.

Submit Application

After completing the application form, you can submit it in person at one of our participating libraries.

Visit the Central Library during passport service hours - no appointment needed.
For Avondale Branch and Springville Road Branch libraries, appointments are required. Please call your preferred location to schedule.

Bring the completed form along with the required documents and payment.

Passport Photograph

One passport photo is required.

It must:

  • Be taken within the past 6 months
  • Be 2 x 2 inches in size
  • Show a front view on a plain white background
  • Include only the applicant in the photo

Photo services are available at the Central, Avondale Branch, and Springville Road Branch libraries for a fee of $15.00.

Payment Information

Two separate payments are required when applying for a passport:

1. Payment to the U.S. Department of State

This covers the passport application fee and any additional service fees (such as expedited processing). This payment must be made by:

  • Personal check
  • Certified check
  • Cashier's check
  • Traveler's check
  • Money order

Note: Make this payment payable to "U.S. Department of State". Credit and debit cards are not accepted for this portion.

2. Payment to the Birmingham Public Library

This covers the execution (processing) fee. It can be paid using one of the following:

  • Cash
  • Credit or debit card
  • Check
  • Money order

New & Renewal Applications

The Birmingham Public Library accepts new passport applications and can assist with the process from start to finish. If you're applying for your first U.S. passport, or your previous passport was issued more than 15 years ago, you'll need to apply in person using Form DS-11.

Please note that there are special requirements for minors under age 16, minors age 16 and 17, and no-fee passports when submitting Form DS-11.

If you're eligible to renew by mail, you can complete your renewal on your own using Form DS-82 and send it directly to the U.S. Department of State.

Requirements

Identification

To apply for a passport, you must provide both proof of U.S. citizenship and proof of identity.

Proof of U.S. Citizenship (bring one of the following):

  • Certified U.S. birth certificate
  • Consular Report of Birth Abroad
  • Certificate of Naturalization
  • Certificate of Citizenship
  • Previously issued, undamaged U.S. passport (if applicable)

Proof of Identity (bring one of the following):

  • Valid driver's license
  • Government-issued ID (city, state, or federal)
  • U.S. military ID
  • Previously issued U.S. passport (if still valid or not too damaged)

Important: You must bring original documents. Photocopies alone will not be accepted. A clear photocopy of each document is also required to submit with your application.

Passport Photos

You must submit one recent passport photo with your application. The photo must meet U.S. Department of State requirements.

Photo Requirements:

  • Color photo taken within the last 6 months
  • 2 x 2 inches in size
  • Plain white or off-white background
  • Full face, front view with a neutral expression or natural smile
  • No glasses, hats, or head coverings (unless for medical or religious reasons with documentation)

Photo Services at the Library

Passport photo services are available at Central Library, Avondale Regional Branch Library, and Springville Road Regional Branch Library for an additional fee. Ask a staff member at your location for availability and pricing.

Tip: Taking your photo at the library ensures it meets all federal requirements and keeps the process simple.

Documents

To apply for your passport at the Birmingham Public Library, you'll need to bring specific documents with you.

Required Documents for Most Applicants:

  • Form DS-11 - Application for a U.S. Passport (for new applicants)
  • Form DS-82 - Adult Passport Renewal Form (only if your most recent passport was issued within the last 15 years)

Special Situations - Additional Forms You Might Need:

These forms are only required in special cases. Ask staff if you're unsure whether you need one.

  • Form DS-64 - For lost or stolen passports
  • Form DS-5504 - For name changes, data corrections, or limited-validity passport replacement (within 1 year)
  • Form DS-3053 - Parental consent form (must be notarized) for a child's passport
  • Form DS-5525 - For special family or exigent circumstances
  • Form DS-86 - For passports not received after mailing
  • Form DS-10 - For delayed birth certificates (must be notarized)
  • Form DS-71 - Identity affidavit with witness (for applicants without valid ID)
  • Form DS-60 - For name changes not reflected on other legal documents
